Registered Nurse; RN - Case Manager

Job Description

Responsible for organizing and providing care to patients through the process of assessment, planning, intervention, and evaluation. Supports the physician and interdisciplinary team to assure patient progress through the continuum of care to discharge, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payers.

The role integrates and coordinates utilization management, care facilitation, and discharge planning functions, and is accountable for planning effectively to meet patient needs, manage length of stay, and promote efficient utilization of resources for a designated caseload.

• Monday‑Friday 8:00 am – 4:30 pm
• Holiday rotation

• Preferred experience in Case Manager or Acute Care Nursing (2‑4 years of experience is ideal).
• BSN or ADN degree from an accredited school or college of nursing.
• Current RN licensure in the appropriate state(s) of employment.
• Fargo:
  Basic Cardiac Life Support (BCLS) not required.

36.01 – 54.02 USD per hour, Full‑Time, FTE 1.
