General

Summary

As a Hospice RN Care Consultant, you will serve as a trusted clinical advisor and community liaison, working across healthcare settings to identify patients appropriate for hospice care, educate providers and partners on hospice services and benefits, coordinate seamless transitions into care, and build strong, lasting referral relationships. This role combines clinical navigation and outreach strategy, ideal for an RN who enjoys both care coordination and community engagement.

Essential

Functions
  • Guide providers and families in understanding hospice eligibility and care options
  • Facilitate hospital referrals, admissions, and care transitions
  • Coordinate with clinical and admissions teams to ensure timely patient access
  • Support documentation and assist with physician orders for admission
  • Provide in‑services, presentations, and one‑on‑one education to healthcare partners
  • Promote understanding of hospice philosophy, benefits, and services
  • Provide education and training to internal team members and partner facility staff to promote collaboration, strengthen clinical best practices, and ensure consistent, high‑quality care delivery across the continuum
  • Serve as a subject matter expert in end‑of‑life care
  • Develop and maintain strong referral partnerships with physicians, hospitals, and community organizations
  • Execute a strategic territory plan to grow hospice awareness and appropriate referrals
  • Analyze referral trends and adjust outreach efforts
  • Represent HIA Health at events, health fairs, and professional meetings
  • Serve as a visible, trusted presence throughout the healthcare community
Collaborate with Marketing, Education, and Clinical Teams

In addition to the functions above, you will collaborate closely with marketing, education, and clinical teams to align outreach initiatives and internal communications.

Pay Range

Starting at $34.67 per hour, based on experience.

Qualifications
  • Active Registered Nurse (RN) license in North Dakota and Minnesota
  • 3–5+ years of nursing experience (hospice, palliative care, or long‑term care preferred)
  • Valid driver’s license and ability to travel locally
  • Experience in hospice, home care, or end‑of‑life care
  • Background in outreach, education, or healthcare marketing
  • Strong public speaking and relationship‑building skills
