POSITION SUMMARY

As a Hospice LPN, you will provide nursing care to hospice patients in coordination with the interdisciplinary team (IDG) that is consistent with the goals, definitions, philosophy, and purpose of HIA Hospice. All nursing care, as approved by patients' primary physicians or the hospice medical director, including physical assessments, symptom management, and emotional support.

This position will be 12-hour night shifts, rotating every 3rd weekend, and two holidays per year.

KEY RESPONSIBILITIES

  • Provide inpatient comfort focused care through assessments, planning, triage, intervention, and evaluation.
  • Complete appropriate assessments for admission to Inpatient Unit based on level of care requirements.
  • Symptom Management:
    Manage pain and symptoms effectively to improve patient comfort and dignity through end of life.
  • Assess the patient and family needs on an ongoing basis while hospice care is being provided inpatient.
  • Develop and/or update individualized plan of care per shift in collaboration with the patient/family, as applicable, and the hospice IDG that identifies needs, expected outcomes, and interventions.
  • Participate in daily rounds with inpatient IDG team to proactively plan for patient/family needs.
  • Implements and utilizes physician orders accurately and effectively.
  • Teach patient/family in areas related to caregiving in preparation for patients to return to prior level of care.
  • Provide compassionate support to patients and families, building trust and understanding along with serving as a trusted liaison for the care team.
  • Begin daily communication with home care team and inpatient LSW upon admission to facilitate efficient coordination for discharge planning.
  • Complete and lock documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.

Pay Range: Starting at $24.35/hour (based on experience) + shift differential

Qualifications

REQUIRED EDUCATION, CERTIFICATIONS, AND EXPERIENCE

  • Minimum of Licensed Practical Nurse (LPN);
  • Minimum of one year of experience as a hospital staff nurse and/or minimum of two years of experience in health care.
  • Current state LPN licensure in North Dakota and Minnesota (or willing to obtain dual licensure following offer)
  • Current driver's license and auto insurance

PREFERRED QUALIFICATOINS

  • Strong clinical and critical thinking skills to meet complex patient needs
  • Strong interpersonal communication, counseling, and teaching skills
  • Flexibility in scheduling; team player
  • Familiarity with principles related to dying and death strongly preferred
