Job Description
The Licensed Practical Nurse (LPN) will provide skilled nursing care primarily in home health care settings. Working under the direct supervision of a registered nurse (RN) or physician, the LPN will manage a low patient caseload of 1 to 5 individuals, ensuring comprehensive and compassionate care. This role offers clear pathways for professional development and advancement within the healthcare team.
Responsibilities
- Deliver direct patient care in home health environments
- Administer medications accurately and safely
- Monitor vital signs and report any changes
- Perform wound care and dressing changes
- Provide patient education to support health management
- Assist RNs with clinical duties and care coordination
- Implement infection control protocols
- Contribute to individualized care planning
- Maintain clear and effective communication with patients and healthcare providers
Preferred Qualifications
- Minimum of 1 year nursing experience
