Occupational Therapist (OTR/L) - Home Health | Transform Everyday Living
Schedule: Full-Time | Monday-Friday
Compensation: $80,000-$150,000 annually

- Perform occupational therapy evaluations in patients' homes.
- Develop individualized treatment plans that improve independence with activities of daily living.
- Assess home safety and recommend adaptive equipment and environmental modifications.
- Educate patients and caregivers on techniques that improve safety, function, and quality of life.
- Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
- Complete timely, accurate documentation while monitoring patient progress.
- Active Minnesota Occupational Therapist (OTR/L) license.
- Strong communication, organization, and clinical reasoning skills.
- Home health experience is preferred but not required.
- Valid driver's license and reliable transportation.
