Social Worker- Public Health

** FIRST REVIEW OF APPLICATIONS OCTOBER 12TH

Provide case management, care coordination, and long-term care screenings to Rice County residents.

• Provide long-term care consultation services in home and community settings; work as part of a long-term care team at Rice County Public Health.
• Assess and evaluate clients' health and living situations to determine level of care and services needed.
• Provide care coordination and case management services.
• Provide relocation assistance including assessment, community support plan development, and referrals as needed.
• Coordinate client care with medical providers, vendors, clients, and families.
• Maintain accurate and up-to-date records.
• Other duties as assigned.

This position requires a Bachelor’s degree in Social Work or related human services field, or equivalent. A valid Driver’s license is required upon hire.

One year of social work experience in home or community-based services. Experience conducting MNChoices assessments and/or waiver case management services. Bilingual: (English/Spanish and/or English/Somali).

HIRING RANGE / UNION: $36.01 - $50.41 per hour / SS Unit – Grade 12

.8 FTE - 1.0 FTE (32 - 40 hours per week). Work hours typically Monday – Friday, between the hours of 8:00 a.m. – 4:30 p.m.
