Job Title:
Field-Based Nurse Practitioner – AGNP / CNP | Wound Care Certification Paid + Independent Role

Compensation: $105,000 to $120,000 annually

Benefits: Full Benefits Package (for full-time status)

Schedule: Full-Time or Part-Time

Position Summary:

We are hiring a field-based Nurse Practitioner (AGNP or CNP) who thrives in an independent, flexible environment.
If you’re not yet certified in wound care, we’ll pay for it. This is a great fit for someone looking to grow their clinical specialty while making a direct impact on a vulnerable population.

Key Responsibilities:

• Visit assisted living, memory care, and senior communities to provide primary care
• Address wounds, chronic conditions, and preventative care in place
• Partner with care teams and families to ensure holistic support
• Keep accurate records and communicate clearly with all stakeholders

Qualifications:

• AGNP certification preferred
• Experience with geriatrics, family practice, or long-term care
• Must have a valid driver’s license and auto insurance
• Strong documentation, collaboration, and communication skills
