Job Summary
The RN Care Coordinator provides patient-centered care through telephone triage, medication management, care coordination, and clinical support in a clinic setting. This role is responsible for assessing patient needs remotely for both adult and pediatric populations, prioritizing care based on acuity, and collaborating with providers to ensure safe, timely, and appropriate care. The RN also supports in-clinic nursing functions as needed.
CompensationHourly wage range: $36.50–$51.53 (base salary). Compensation is determined based on experience, education, and internal equity considerations.
Key Accountabilities Telephone Triage (Primary Role)- Respond to incoming patient calls for both adult and pediatric populations
- Perform structured symptom assessment using established triage protocols
- Identify urgent or emergent conditions and direct patients to the appropriate level of care (e.g., same-day visit, urgent care, emergency department)
- Provide evidence-based guidance and home care instructions when appropriate
- Clearly document all triage interactions, recommendations, and disposition in the EHR
- Escalate complex or unclear cases to the provider for guidance
- Triages walk-in patients for level of care needed.
- Process medication refill requests in accordance with clinic protocols and standing orders
- Review patient charts, labs, and visit history to ensure refill appropriateness
- Identify when provider review is required and route accordingly
- Communicate with patients regarding refill status, requirements (e.g., labs, follow-up visits), and medication instructions
- Ensure timely completion to support continuity of care and patient safety.
- Coordinate patient care across providers, departments, and services
- Facilitate referrals, prior authorizations, and follow-up care
- Track and manage test results, ensuring timely communication to patients
- Support transitions of care (e.g., post-hospital discharge follow-up calls)
- Assist with chronic disease management and preventive care outreach.
- Support delivery of safe, high-quality, and patient-centered care that is age-appropriate, developmentally appropriate, and culturally sensitive
- Assist in maintaining consistent clinical standards and workflows across the clinic
- Collaborate with team members to address workflow challenges, remove barriers, and improve care coordination
- Participate in quality improvement initiatives, including development and refinement of protocols, procedures, and policies
- Provide informal education and support to team members to promote clinical growth and consistency
- Serve as a professional role model by demonstrating accountability, teamwork, and excellent patient service
- Promote and maintain a safe environment for patients, families, and staff.
- Perform patient intake, including vitals, medication reconciliation, and focused assessments
- Administer medications, vaccines, and treatments per provider order
- Assist with minor procedures and point-of-care testing
- Provide patient education on diagnoses, medications, and care plans
- Work closely with providers, clinic staff, and external partners
- Deliver clear, compassionate education tailored to patient needs and health literacy
- Reinforce provider instructions and follow‑up plans
- Support patient engagement and adherence to treatment plans.
- Accurately document all patient interactions in the EHR
- Adhere to organizational policies, clinical protocols, and regulatory standards
- Maintain confidentiality and HIPAA compliance.
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education skills
- Ability to prioritize, multitask, and work independently.
- Current and unrestricted licensure as an RN in the State of Minnesota.
- Basic Life Support certification per American Heart Association guidelines within 6 months of hire.
- 3 years of clinical experience in a clinical/patient care environment.
- Must be able to speak, read, and write English proficiently.
