Job Title: Full-Time Wound Care Nurse Practitioner (AGNP/CNP) | Senior Living Rounding Expert
Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Schedule: Full-Time | Monday through Friday
Responsibilities- Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
- Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
- Manage medications, diagnostics, treatment plans, and follow-up care.
- Collaborate with physicians, nurses, therapists, caregivers, and facility leadership.
- Identify barriers to healing and implement evidence-based interventions.
- Support continuity of care while helping improve clinical outcomes.
- Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
- Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
- Strong clinical judgment and confidence managing medically complex patient populations.
- Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
