What You'll Do Provide skilled nursing visits in patients' homes. Perform wound care, medication administration, and disease management. Reinforce education provided by the RN Case Manager. Monitor patient conditions and communicate changes to the care team. Collaborate with physicians, therapists, and fellow clinicians. Complete accurate and timely documentation.
What We're Looking For Active Minnesota Licensed Practical Nurse (LPN) license. Reliable transportation with a valid driver's license and current insurance. Ability to work independently and manage your daily schedule. Previous nursing experience in skilled nursing, rehab, TCU, hospice, or home health is preferred. Strong communication skills and a passion for patient-centered care.
