Compensation: $30 - $38 per visit

Home Care is in high demand and we are looking for experienced and caring LPNs to join our team at Home Health Care Inc. Successful applicants will have a proven track record as an LPN. Home care experience is a huge benefit but not required.

• Provide patient care according to client’s care plan and needs.
• Perform wound care treatments and assist clients with medication management.
• Teach the client and/or family members about care and support skills.
• Communicate regularly with RN Case Managers, Supervisors, and the Care Team.

• Licensed as a Practical Nurse / LPN in the state of Minnesota.
• Driving is required; must have a reliable vehicle with current insurance and a valid Driver’s License.
• Demonstrated ability to work autonomously.
• Prior nursing experience in a TCU, Hospice, or Home Health setting.
• Experience with Electronic Medical Records (EMR) software is preferred.
• Military Occupational Classification (MOC) Codes:
  Army – 68C;
  Navy – HM;
  Air Force – 4N0X1;
  Marines – HM;
  Coast Guard – HS.
