Your patient visits will be scheduled within approximately a 35-mile radius of your home
, allowing you to serve patients in communities closer to where you already live.

What Will You Actually Do?

Your visits may include:

• Skilled nursing assessments and observations
• Wound care and dressing changes
• Medication management and education
• Injections and other ordered nursing treatments
• Monitoring chronic conditions and changes in health status
• Reinforcing physician orders and treatment plans
• Teaching patients and caregivers about medications, diagnoses, and disease management
• Identifying concerns and communicating changes to the RN Case Manager or physician
• Collaborating with therapists and other interdisciplinary team members
• Maintaining accurate and timely clinical documentation

Most importantly, you’ll help patients continue receiving the skilled nursing care they need without leaving home.
