The Case Manager provides comprehensive case management services for Ruth’s House (RH), an emergency and transitional shelter serving up to nine families, and Sarah’s House (SH), a recovery residence serving up to five women. The Case Manager supports residents across both programs and helps create safe, stable, and supportive environments that promote long-term housing, recovery, independence, and personal well-being.

Using a trauma-informed, strengths-based, and person-centered approach, the Case Manager works collaboratively with residents to develop individualized service plans, identify barriers, establish goals, and implement strategies that support stability and effective crisis management. The Case Manager serves as a primary resource and advocate for residents and helps connect them with appropriate community resources and services.

This is a highly relational position requiring strong communication, networking, advocacy, and collaboration skills. The Case Manager develops and maintains partnerships with local service providers, treatment facilities, hospitals, government agencies, and other community resources throughout Minnesota to support residents and promote awareness of RH and SH programs.

The Case Manager is a member of the Leadership Team, reports directly to the Program Director, and

participates in shared organizational leadership and decision-making.

Key

Responsibilities and Duties:

Resident Case Management & Support Admissions, Documentation & Program Compliance
  • Follow established admissions and intake procedures for both RH and SH programs.
  • Complete intake and required documentation in the case management software (CMS).
  • Follow GRH funding and transfer procedures and submit required information to the County in a timely manner.
  • Communicate proactively with county and government agencies regarding resident benefits, including Housing Support and other assistance programs.
  • Monitor deadlines and ensure required forms and documentation are complete and submitted accurately and on time.
    Adhere to all program policies, procedures, resident agreements, and expectations.
  • Coordinate with the Program Director when significant policy violations or concerns arise. The Program Director determines when a resident is asked to leave programming, and the Case Manager will participate in reviewing the situation and developing an appropriate transition plan.

Qualifications & Requirements
  • Bachelor’s degree in social work, psychology, human services, or a related field preferred; equivalent relevant education and experience may be considered.
