LPN Pediatric Home Care Night Shift

Role Overview:

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions.

• Deliver high-quality clinical services within legal and care plan guidelines, adhering to infection control protocols.
• Document patient visits accurately in electronic medical records and electronic visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.

• Current unrestricted LPN licensure and CPR certification.
• Valid driver's license, dependable vehicle or transportation access.
• Ability to work with diverse populations and adapt to various home environments.
• Preferred:
  Home care experience, strong communication, organizational skills, and ability to work independently.

Compensation ranges from $20.50 to $30.75 per hour, with benefits including health coverage, incentives, and retirement plans. The role emphasizes compassionate, patient-centered care and effective team collaboration.
