Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse 6a-6p (Mon-Thurs) to join our team at Onyx Care of Farmerville, a premier post-acute rehabilitation center in Farmerville, LA. As a key member of our healthcare team, you will play a vital role in providing exceptional care to our patients. Our patient-centered approach focuses on individualized attention, innovative technology, and around-the-clock skilled nursing care.

• Provide high-quality, patient-centered care to our residents, promoting their physical, emotional, and psychological well-being

• Administer medications, treatments, and therapies as prescribed by our medical team

• Monitor and report patient progress, identifying areas for improvement and developing personalized care plans

• Collaborate with our interdisciplinary team to ensure seamless care transitions and effective communication

• Maintain accurate and detailed records of patient care, medication administration, and treatment outcomes

• Perform routine tasks, such as vital sign monitoring, wound care, and assistance with activities of daily living (ADLs)
