We are seeking an experienced and compassionate Registered Nurse (RN) – Home Health/Hospice to provide high-quality skilled nursing care to patients in their homes. The RN will work independently in a community-based setting, delivering comprehensive assessments, clinical interventions, patient education, and care coordination while ensuring compliance with regulatory and agency standards.

This role requires strong clinical judgment, excellent communication skills, and the ability to manage a diverse caseload across multiple locations.

• Comprehensive In-Home Patient Assessments
  • Conduct initial and follow-up patient assessments in the home setting.
  • Evaluate physical condition, vital signs, symptoms, and overall health status.
  • Assess medication compliance and effectiveness.
  • Identify home safety concerns and environmental risks.
  • Evaluate patients' ability to perform Activities of Daily Living (ADLs).
• Development and Management of Care Plans
  • Develop individualized plans of care based on patient assessments.
  • Ensure care plans align with physician orders and regulatory guidelines.
  • Review and update care plans as patient conditions change.
  • Establish measurable goals and interventions to optimize patient outcomes.
• Direct Skilled Nursing Care
  • Administer medications, injections, and IV therapy as ordered.
  • Perform wound care and monitor healing progress.
  • Provide post-hospitalization and post-surgical follow-up care.
  • Deliver chronic disease management for conditions such as diabetes, COPD, and heart failure.
  • Monitor and manage symptoms related to hospice and palliative care when applicable.
• Ongoing Patient Monitoring and Clinical Evaluation
  • Evaluate treatment effectiveness and patient progress.
  • Recognize early signs of complications or clinical deterioration.
  • Communicate patient status changes promptly to physicians and interdisciplinary team members.
  • Implement appropriate interventions based on clinical findings.
• Patient and Caregiver Education
  • Educate patients and families regarding:
    • Medication management and side effect monitoring
    • Disease processes and symptom recognition
    • Self-care techniques that promote independence
    • Infection prevention and home safety practices
  • Reinforce treatment plans and health maintenance strategies.
• Care Coordination and Interdisciplinary Collaboration
  • Collaborate with physicians, therapists, social workers, and home health aides.
  • Coordinate services and facilitate referrals as patient needs evolve.
  • Ensure continuity of care across all disciplines.
• Clinical Documentation and Compliance
  • Complete accurate and timely clinical documentation using agency EMR systems.
  • Maintain compliance with Medicare, CMS, state, and agency regulations.
  • Support quality improvement initiatives, audits, and clinical reviews.
• Patient Advocacy and Emotional Support
  • Support patients and families through challenging health situations.
  • Advocate for patient needs, preferences, and goals of care.
  • Promote dignity, autonomy, and overall quality of life.
• Independent Clinical Decision-Making
  • Manage daily schedules and prioritize patient visits effectively.
  • Exercise sound clinical judgment in home-based settings.
  • Respond appropriately to urgent or emergent patient situations.

• Active, unrestricted Registered Nurse (RN) license in the state of practice.
• Current Basic Life Support (BLS) Certification.

• Minimum of 1 year of recent Home Health experience required.
• Experience using Home Care Home Base (Point Care) EMR system required.

• Ability to work independently in patient homes with minimal supervision.
• Strong assessment and clinical decision-making skills.
• Valid driver's license.
• Reliable transportation for daily travel between patient homes.
