Field-Based (Jackson / Washtenaw Corridor)

- Concord (as needed)
- Conduct comprehensive nursing assessments
- Develop individualized plans of care
- Deliver skilled nursing interventions in the home
- Educate patients and families on condition management and medications
- Coordinate with physicians, therapists, and interdisciplinary team members
- Complete accurate and timely documentation in Well Sky / Kinnser
- Utilize Home Health Notify for real‑time communication and updates
- Ensure compliance with state, federal, CHAP, and Joint Commission standards

Help patients recover and thrive safely at home
- Graduated from an accredited school of nursing
- Hold an active Michigan RN license
- Have at least 2 years of nursing experience
Home health experience preferred - Are compassionate, organized, and resourceful
- Have strong assessment and communication skills
- Have reliable transportation, valid auto insurance, and a driver’s license
