Caretenders Hospice, part of the LHC Group and Optum family, is seeking a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients. You will collaborate with the care team to promote well-being across physical, psychosocial, emotional, and spiritual dimensions.
Primary responsibilities include initiating the initial nursing evaluation within 48 hours, coordinating the plan of care, documenting progress, and guiding patients and families in self-care.
