As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions- reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration
  :
  Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action
  :
  Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop
  :
  Complete documentation/charts within the same day
• High-volume rounding
  :
  Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback
  :
  Surface field insights to improve workflows and tech integration in a clinical setting
• Mentor & collaborate
  :
  Support NP onboarding, share best practices, and partner with clinical ops/product teams.
• Regional flexibility: Cover sites within a 30-40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.
