T.O.N.E. Home Health Services, Inc. is seeking a compassionate and experienced Psychiatric Nurse to join our growing clinical team in a field-based role providing in-home psychiatric nursing care to patients throughout Southeast Michigan.

This position is ideal for a nurse who enjoys direct patient interaction, independent clinical decision-making, and collaborating closely with interdisciplinary healthcare teams to improve behavioral health outcomes in the home setting.

We are looking for someone who is clinically strong, responsive, technology-comfortable, and passionate about delivering patient-centered psychiatric care where it matters most: in the patient’s home.

Part-Time / Per Diem

Flexible scheduling based on patient census and visit availability

Visit-Base Pay

Competitive per-visit rates based on experience

• Conduct psychiatric nursing visits in patient homes
• Assess and monitor patients experiencing anxiety, depression, behavioral health concerns, dementia-related behaviors, and other psychiatric conditions
• Perform nursing assessments and symptom monitoring
• Provide medication education and monitor treatment compliance
• Collaborate with physicians, nurse practitioners, therapists, social workers, and interdisciplinary team members
• Educate patients and families regarding behavioral health care plans
• Identify changes in patient condition and communicate concerns promptly
• Complete timely and accurate documentation within Kinnser/Well Sky EMR
• Support continuity of care through strong communication and follow-up
• Participate in interdisciplinary care planning as needed

• Current Michigan RN license
• Psychiatric nursing experience required
• Minimum 2 years of nursing experience required
• Home health or community-based care experience strongly preferred
• Strong clinical assessment and critical thinking skills
• Comfortable with technology and EMR documentation
• Excellent communication and patient education skills
• Valid driver’s license, reliable transportation, and current auto insurance
