STAR is looking for a motivated and passionate PER DIEM Physical Therapist to treat patients in the Home setting around the "put your geographical area here" area. Job Description- Accept and manage patients within your designated geographical area of coverage
- Plan, prepare, and implement customized therapy programs designed to restore overall function and promote independence in the home setting
- Develop and maintain individualized care plans/goals in collaboration with physicians, patients, and their families
- Ensure timely and accurate documentation of all patient visits and therapy services
- Effectively communicate discharge planning with patients, families, and interdisciplinary team members
- Maintain compliance with all clinical and documentation standards
- Physical Therapist Program Graduate.
- Arkansas Physical Therapist License or eligible.
- Liability insurance
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
