Capacity Management Center Nurse Spec,

At UConn Health, this advanced working level class is accountable for performing a range of tasks in patient capacity management, ensuring safe, efficient, clinically appropriate and timely use of hospital services to optimize patient care and experiences.

Schedule

Full time, 40 hours per week; 7:00 a.m. to 3:30 p.m., every other weekend and rotating holidays; 30‑minute unpaid meal break.

Reports to:

employee of a higher grade; leads employees of a lower grade as assigned.

Examples of Duties
  • Coordinate all incoming patient transfer requests from a variety of referring sources, including internal and external medical facilities such as hospitals, clinics, urgent care centers, emergency departments, and outpatient providers.
  • Conduct rapid clinical assessments and determine appropriate level of care for patient placement upon transfer or direct admission.
  • Facilitate communication between referring providers and UConn Health accepting physicians to secure physician‑to‑physician acceptance and ensure accurate documentation of clinical rationale.
  • Identify, assess and prioritize time‑sensitive or emergency conditions requiring immediate escalation.
  • Serve as a central communication hub for patient placement, coordinating with Bed Management staff, nursing supervisors, service line leaders and hospital operations to identify appropriate bed placement for routine operations and during capacity constraints.
  • Interpret, apply and ensure compliance with UConn Health policies, procedures, state and federal laws, regulations and standards, including the Emergency Medical Treatment and Labor Act (EMTALA).
  • Utilize electronic and other computerized software, applications, modules and other technologies such as the Transfer Center module to facilitate transfers; research, document, analyze, track and report on relevant data; perform ad hoc audits to meet operational and regulatory needs.
  • Participate in initiatives to improve patient placements in appropriate care settings at the right time while optimizing hospital capacity; analyze trends and suggest process and/or case coordination improvements.
  • Perform related duties as required.
Minimum Qualifications
  • Graduation from an accredited nursing program.
  • Three (3) years of professional nursing experience in an inpatient setting such as the Emergency department, Intensive Care Unit, Step‑Down/Intermediate unit.
  • Will have an active nursing license in good standing in the state of Connecticut.
Knowledge, Skills, and Abilities
  • Considerable knowledge and ability to apply nursing principles, practices, theory, ethics, and methods.
  • Considerable knowledge and ability to apply clinical standards of care, documentation and patient record policies, practices and standards.
  • Considerable knowledge and ability to interpret and apply University policies, procedures and best practices as well as federal and state laws, regulations and standards.
  • Considerable knowledge and ability to apply clinical assessment and triage principles and practices.
  • Considerable knowledge and ability to apply hospital capacity management principles, practices and methods.
  • Considerable knowledge and ability to use a computer, including a range of software, applications and other technologies.
  • Considerable verbal and written communication skills.
  • Considerable time‑management skills, including attention to detail.
  • Considerable ability to think critically and to solve problems.
  • Considerable ability to work under pressure, managing and balancing multiple competing priorities.
Preferred Qualifications

Critical care or step‑down level care experience.

Working Conditions

Incumbents may be exposed to some disagreeable conditions, stress and demands for critical or urgent actions. Employees must work in person for long hours, nights, weekends and holidays to meet operational needs.
