Interim Health Care - Farmington, CT is seeking a Weekend Home Health Licensed Practical Nurse (LPN) for a 30-hour position in the Greater Hartford Area. This role focuses on one-on-one home care, coordinating with an RN and health team to deliver quality, compassionate care.
You will take vitals, perform assessments, document progress, and educate patients and families on care plans, medications, and home safety. Must be CT-licensed LPN with CPR and knowledge of home health regulations.
