Float Pool Licensed Practical Nurse

Sandy River Center, a Genesis Health Care skilled nursing and rehab facility in Farmington, ME, seeks a Float Pool Licensed Practical Nurse. The LPN provides compassionate bedside care across units, administering medications and treatments, monitoring residents, and updating care plans. Collaborating with RNs, CNAs, and therapists, you'll support short-term rehab, long-term care, and memory support residents. Responsibilities

• Provide direct nursing care to residents across multiple units as part of the float pool
• Administer medications and treatments per provider orders and facility protocol
• Monitor resident conditions, perform assessments, and promptly report changes to RNs or providers
• Document care, observations, and interventions accurately in the EHRCoordinate with RNs, CNAs, therapists, and other team members on individualized care plans
• Support short-term rehab, long-term care, and memory support residents with resident-centered focus
• Educate residents and families on care plans, medications, and health management
• Maintain infection control, safety, and quality standards in all clinical areas
• Participate in interdisciplinary team meetings, trainings, and quality initiatives
• Assist with orientation and support of new staff as needed
