Adult Community Case Manager

LEAP is seeking an Adult Community Case Manager to support adults with disabilities in Allegan County. In this role, you will provide community-based case management, including intake, assessment, and person-centered planning. You will coordinate services with healthcare providers, social service agencies, and community resources to promote independence, inclusion, and quality of life. Responsibilities include advocacy, regular home and community visits, progress monitoring, and accurate documentation.
