Social Worker – MHMG
The Social Worker – MHMG role provides professional social work services that support the goal of the Population Health program to deliver risk-based population management services to patients at all times during their tenure of care.
Required Minimum Knowledge, Skills, and Abilities (KSAs)
- Education:
Bachelor's Degree in Social Work from an institution accredited by the Council on Social Work Education required. - License/
Certifications:
Active license by the Maine State Board of Social Work required. - Experience:
Experience providing therapeutic services to adult and pediatric patients with chronic medical issues required. Experience in client advocacy and case management required. - Additional Skills/Requirements

The base range is an estimate and does not reflect the full value of our total compensation package.
The pay range for this position is $61,256.00 to $82,867.20 per year.
Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
