The Registered Nurse – Home Health position allows you to provide high‑quality, 1:1 patient care in the patient’s home, supporting over 60,000 patients each day across our areas of coverage.
Job OverviewAs a Registered Nurse Home Health, you will work autonomously while collaborating with an interdisciplinary team to deliver personalized care that aligns with Elara Caring’s vision and HIPAA privacy requirements.
Responsibilities- Ensure all activities performed align with Elara Caring’s board of directors, executive team, and Home Health leadership.
- Maintain patient and staff privacy and confidentiality pursuant to HIPAA.
- Assess assigned home‑health caseload to identify physical, psychosocial, and environmental needs of patients and families.
- Assume primary responsibility for the caseload – including assessment, planning, coordinating, implementing, and evaluating the plan of treatment.
- Conduct initial and follow‑up nursing evaluations for each visit, gathering data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physicians, facilities, and other personnel involved in patient care.
- Revise the plan of care in response to identified issues and notify the Clinical Supervisor and other team members.
- Current, unrestricted state RN license.
- 1+ year clinical nursing experience.
- Ability and willingness to travel within the branch/office coverage area; 50% travel expected.
- Ability to lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Associate’s or Bachelor’s degree in Nursing preferred.
- Previous Home Health or Hospice experience preferred.
- Previous experience with Home Care Home Base preferred.
- Will report to the Clinical Manager or Clinical Supervisor.
