• Board Certification: Required
• License: Active VA license required
• Certifications: BLS, PALS, ACLS
• Provider Must be Local: Only Mileage is Reimbursed - note at the time of name clear

• Average Number of Patients Per Day: 18
• Number of Exam Rooms: 16
• Services Available On-Site: Laboratory and Radiology
• Documentation System/EMR: EPIC
• Emergency Supplies Available: Defibrillator
• Required General Procedures: Evaluation and Mgmt. of Acute Volume / BP Issues
• Required Medicine Procedures: Adult Outpatient and Without ICU/CCU
• Required Pediatric Procedures: General Outpatient
• Required Gynecology Procedures: Pelvic Exam / Pap Smear
