Full Time Wound Care Nurse Summary & Objective

In keeping with our organization’s goal of improving the lives of the residents we serve, the Wound/Treatment Nurse plays a critical role in providing superior customer service and nursing care to all residents. The Wound Nurse is responsible for performing skin treatments for all residents under their care, obtaining treatment orders from attending physicians, and modifying the treatment regimen in accordance with established policies and procedures.

The Treatment Nurse will provide supervision of staff and safeguard the health, safety, and welfare of all guests under their care by following applicable laws, regulations, and established nursing policies and procedures.

Daily

Complete a head‑to‑toe body audit on all new admissions and re‑admissions to ensure that all open areas have been identified by the admitting nurse. Please ensure that skin sheets, treatment orders are written, and preventative measures are put in place (T&P, specialty mattress, heels up ordered or boots).

  • Review residents identified with new skin alterations and ensure implementation of skin sheet, treatment, specialty mattress, Braden scale updated, T&P orders, heels up orders or boots (if applicable) and initiate care plan (UM).
Weekly
  • Update skin sheets every 7 days on a specific day and ensure completion by the close of business.
  • Unit managers update care plans weekly for pressure and non‑pressure areas; care plans should be updated every week by Friday.
  • Send wound report to VP of Clinical Services by Thursday with the percentage of Center‑Acquired Pressure Ulcers. Remember our goal is 3% or less.
  • Complete treatments for vascular, surgical, and pressure of stage 2 or greater Monday‑Friday.
  • Treat skin tears, stage 1, through the charge nurse Monday‑Friday, completing skin sheets weekly. The unit manager and wound nurse must ensure these sheet reviews.
On‑Going
  • Prevent wounds: daily education to staff, reminders to T&P residents (in wheelchairs, geri‑chairs and in bed), toileting, incontinent care. A proactive, reactive approach to wound care will make the facility successful.
  • When wounds worsen, change treatment within a 2‑week period with input from the wound physician and PCP.
  • Complete preventative items for residents at risk of developing Center‑Acquired Pressure Ulcers, including orders for T&P, barrier cream (with zinc if beneficial), pressure‑relieving device to chairs, gel cushion at minimum in wheelchair/geri chair, Braden assessments on new admissions, quarterly, and when condition changes. If Braden scores lower than a specialty mattress, apply it.
  • Perform rounds by unit managers/charge nurses and wound nurse to ensure proper positioning, T&P q 2 hours and PRN in bed and wheelchairs/geri chairs, heels elevated, and no skin‑to‑skin contact.
  • Consult with the nursing team regarding assessment evaluations and assist in planning skin‑care treatment.
  • Initiate requests for consultation or referral.
  • Assess residents and their records on admission, discriminate between normal and abnormal findings, and determine when to refer to a physician.
  • May act as a unit nurse when directed.
  • Supervise CNAs and subordinate staff, checking their work for completeness.
  • Administer medications within R.N. or L.P.N. scope, maintain current narcotic report.
  • Chart directly during the shift, including detailed monthly evaluations of each resident.
  • Ensure residents' care plans are followed and assess each guest’s status.
  • Round with wound physician weekly and PRN with physicians.
