Position SummaryWe are seeking a dedicated Registered Nurse (RN) to provide skilled nursing services, monitor client health, and ensure high‑quality care according to individualized plans. This role collaborates with RN Case Managers, LPN's, caregivers, and interdisciplinary teams to promote health, safety, and independence.
Key Responsibilities- Perform skilled nursing assessments and implement care plans.
- Administer medications and treatments safely and accurately.
- Complete clinical documentation and report changes in client condition promptly.
- Communicate effectively with clients, families, and care teams to ensure continuity of care.
- Registered Nursing License in state of Tennessee.
- Active RN license in good standing in Tennessee.
- Minimum 1 year of nursing experience preferred; home health experience a plus.
- Strong clinical judgment, documentation skills, and ability to work independently.
- Flexible scheduling options (Full-Time or Part-Time).
- Supportive team environment with ongoing training and skills development.
