Weekend LPN Job Overview
Title:
LVN/LPN — Non-Exempt
Under limited supervision, the licensed practical/vocational nurse (LPN/LVN) provides patient care to assigned patients by applying the nursing processes of assessment, planning, implementation, and evaluation in accordance with federal, state, and local laws, facility policies, and regulatory standards.
Responsibilities- Provides skilled nursing as ordered on the plan of treatment and nursing care plan.
- Each shift of visit reflects the provision of skilled nursing care rendered in accordance with physician’s orders.
- New problems, changes, and/or resolution or problems are identified and followed through with Director/Supervisor and Physician notification, obtaining necessary orders.
- Documents all collaboration and interventions in clinical notes, supplemental physical orders, and nursing care plan.
- Observes aseptic technique as required.
- Performs specific treatments and medication administration as per the care plan.
- Functions as a team member under the direction of the registered nurse.
- Skilled observations and significant changes in patient status are communicated to the Director of Clinical Services and/or Clinical Manager.
- Requests modifications or additional orders based on clinical status and skilled nursing judgment.
- Adheres to MGA policies, procedures, and handbook.
- Completes clinical documentation accurately and submits it timely per agency policy/guidelines.
- Participates in Agency’s mandated activities, such as In Services, as requested.
- Represents themselves positively and professionally in the company and community, adhering to dress code.
- Reports on time and completes work within the specified schedule.
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or postgraduate education relevant to the role.
- Valid/unencumbered LPN/LVN license in the state where services are provided.
- Professional experience as required per state regulations.
- Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
- Reliable transportation.
- Current TB results or chest X‑ray.
- Current CPR certification per agency policy.
- Flexible, organized, and capable of exercising independent and sound judgment to perform the duties.
Ability to sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms as required. Vision and hearing within normal limits, with or without aid. Ability to frequently lift or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable. Must perform the anticipated level of physical assistance required for safe client care;
if the level exceeds safe capacity, supervisor must be notified immediately.
Work is primarily home‑based in private homes with variable lighting, acoustics, air quality, temperature, and environmental hazards. Exposure to wet or humid conditions, household pets, fumes, airborne particles, and caustic or toxic chemicals is possible. Frequent travel to complete home health visits is required.
