BAYADA Home Health Care is seeking a Home Health Physical Therapist for the Fayetteville, North Carolina location. This role involves performing home health visits for adult and geriatric clients across Cumberland, Harnett, and Hoke counties.
Responsibilities- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance, balance testing, and safety assessments.
- Develop and implement individualized care plans with manual therapeutic exercises, gait training, balance, and other interventions.
- Continuously assess and revise the care plan and participate with nursing in the multidisciplinary plan as appropriate.
- Educate and instruct clients, family members, or other representatives in rehabilitative care and activities for health, safety, and independent living.
- Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.
- Current North Carolina Physical Therapist license.
- Graduation from an APTA or AMA‑approved program.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem‑solving skills.
- Solid computer skills; prior EMR experience preferred.
