Essential Functions
  • Delivery of Patient Care
    • Assumes responsibility for the coordination and delivery of patient/caregiver care.
    • Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
    • When appropriate, develops, implements, evaluates, and updates the plan of care to ensure uninterrupted care and achievement of goals.
    • Facilitates patient and caregiver independence to the extent possible.
    • When providing hospice care, appropriately utilizes VIA Health Partners patient‑related protocols, standing orders, and guidelines.
    • Assumes responsibility for conducting effective patient and caregiver education.
    • Encourages and allows time for questions and consultation.
    • Provides education to patients and families as needed.
    • Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
    • Maintains open lines of communication with supervisor, keeping them apprised of any complex patient/caregiver issues.
    • Applies principles of infection control and follows universal precaution guidelines.
  • Interdisciplinary Group
    • Attends, participates, and may facilitate interdisciplinary group conferences (IDG) and special care conferences.
    • Establishes and maintains effective working relationships with team members.
    • Identifies specific patient/caregiver needs and refers to appropriate team members.
  • Documentation
    • Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
  • Professional and Personal Accountability
    • Demonstrates competency in knowledge and skills.
    • Maintains VIA Health Partners professional reputation and projects it positively.
    • Assumes responsibility for professional development and stays abreast of current healthcare trends.
    • Maintains current professional licensure, CPR certification, and CEU requirements.
    • Maintains productive and professional relations with external agencies and healthcare providers.
    • Exhibits time‑management skills and can respond appropriately to crises.
  • May perform other duties as required
    • Cleans patient rooms per patient‑room cleaning procedure.
    • Ensures all areas of the facility are clean and stocked with supplies.
    • Keeps the family room clean throughout the day.
    • Uses proper chemicals and protective equipment when cleaning surfaces.
Minimum Qualifications
  • A diploma or associate degree in nursing is required; bachelor’s degree preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
  • The Nurse Licensure Compact (NLC) allows an RN to practice in other compact states while complying with state practice laws and discipline.
  • Current CPR required; CHPN preferred.
  • Minimum 1‑year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life‑limiting illnesses is required. Related experience may include hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical‑surgical.
  • Computer proficiency is required.
