We are a mission-driven, independent primary care practice serving Fayetteville and surrounding communities. Our patients include elderly individuals with complex needs, people managing chronic conditions, and underserved populations who have too often been failed by the broader healthcare system. We exist to fill that gap with consistency, compassion, and clinical depth.
We are seeking a full-time Nurse Practitioner or Physician Assistant to join our onsite team immediately. This is a long-term role for a clinician who wants to put down roots, build real patient relationships, and practice medicine the way they envisioned it in school.
This role is built for clinicians who want to know their patients by name. It is not the right fit if you are looking for a high-volume shift-based model or a short-term position before relocating.

About the practice
Founded by Daniel
C. Uba, Rapha Health has served the Fayetteville community since 2012. We operate on a biopsychosocial model of care treating the full person, not just the chief complaint.
Our practice of approximately 6,000 active patients reflects the diversity of Fayetteville itself: families, elderly patients, veterans and military households from the Fort Liberty community, individuals managing complex chronic conditions, and patients who have historically lacked access to consistent, high-quality primary care. Serving this population is not incidental to what we do it is why we exist.
Our team includes three providers supported by a full clinical staff. Physicians are available for consultation and collaboration daily the oversight model here is designed to support your practice, not constrain it.
Clinical responsibilities
- Provide comprehensive primary care to adult and family medicine patients
- Diagnose, treat, and manage acute and chronic medical conditions
- Deliver preventive, whole-person, relationship-based care
- Perform basic clinical procedures and diagnostics including X-ray, ultrasound, and EKG
- Document encounters accurately in eClinical Works
- Collaborate with physicians, NPs, PAs, and clinical staff
- Contribute to workflow and practice improvement as a valued team member
Practice environment
- ~25 patients per day with 15-minute appointments and a fully supported clinical team structured to give you time to actually practice the way you were trained

Qualifications
Required:
- Nurse Practitioner or Physician Assistant with active North Carolina licensure
- Primary care experience preferred in Family Practice or Internal Medicine
- Strong clinical judgment and patient-centered communication
- Collaborative practice agreement eligibility (NPs) or comfort working in a team-based model (PAs)
- Comfort working in an EMR environment
Preferred:
- Mission-driven and community-minded
- Experience serving elderly, chronically ill, or underserved populations
- Familiarity with military or veteran patient populations
