Role Description
The RN Case Manager is a full-time, on-site role based in Fayetteville, NC, responsible for coordinating, planning, and managing patient care across the continuum of services. This role conducts comprehensive nursing assessments, develops and updates individualized care plans, and collaborates with an interdisciplinary team to support optimal clinical outcomes. The RN Case Manager works closely with patients, families, and caregivers to provide education, support transitions of care, and ensure services are delivered in accordance with clinical standards, regulatory requirements, and patient goals.
Daily responsibilities include monitoring patient progress, documenting in the medical record, communicating with physicians and other providers, and participating in care conferences and quality improvement initiatives. The role also involves advocating for patient needs, facilitating resources, and promoting safe, compassionate, and efficient care.
- Current, unencumbered Registered Nurse (RN) license in the state of North Carolina (or eligibility to obtain) and current Basic Life Support (BLS) certification.
- Demonstrated clinical nursing skills in acute, post-acute, home health, hospice, or long-term care settings, with strong assessment and patient care planning abilities.
- Experience in case management, care coordination, or discharge planning, including managing caseloads and navigating community and post-acute care resources.
- Proficiency in documentation and electronic health records (EHR), with strong organizational, time-management, and follow-through skills.
- Excellent communication, counseling, and interpersonal skills to collaborate effectively with patients, families, and interdisciplinary team members.
- Knowledge of regulatory and compliance standards relevant to skilled nursing, home health, hospice, or related care settings.
- Ability to work independently and as part of a team in a fast-paced on-site environment, demonstrating sound clinical judgment and problem-solving skills.
- Preferred:
Prior experience with geriatric populations, rehabilitation services, or chronic disease management; certification in case management (e.g.,).
