We're seeking an experienced PA/NP to join a well-established PACE program in Fayetteville, North Carolina, providing comprehensive, patient-centered care to the geriatric population within a collaborative interdisciplinary environment.

The Role

Fayetteville, NC PACE Program | Geriatric Medicine EMR Supported Environment Permanent Position

What You'll Be Doing

  • Provide primary medical care in collaboration with PACE physicians
  • Perform initial, annual, semi-annual, and as-needed participant assessments
  • Develop individualized care plans within the interdisciplinary team model
  • Evaluate acute illnesses and manage chronic conditions
  • Deliver preventive care, screenings, immunizations, and monitoring
  • Prescribe medications and therapies appropriately
  • Coordinate specialist referrals and transitions of care
  • Participate in discharge planning
  • Educate patients, families, and caregivers
  • Contribute to staff education and clinical policy development
  • Maintain accurate and compliant clinical documentation
  • Participate in on-call rotation responsibilities

Candidate Requirements

Graduate of an accredited PA program Active North Carolina PA License or willing to obtain Minimum 2 years PA experience required Geriatric experience preferred Experience with frail/elderly patients or willingness to complete PACE training BLS Certification required DEA Registration required Strong clinical and independent decision-making skills NCCPA Certification required Clean malpractice/license history highly preferred NPDB Self Query Report within 30 days required at presentation No visa sponsorship available No fresh graduates Not accepting locums or locums-to-perm candidates
