Nurse Practitioner - Long-term Care

Pride Health is hiring a Nurse Practitioner in NC! (Contract)

Working between 2 LTC facilities (Fayetteville and Brunswick County, NC)

Schedule
:
Monday-Friday (8am-5pm)

Contract
: 6 month contract to hire

Pay Rate
: $65-70/hour

• Provide specialized care to high-risk health plan members
• Conduct comprehensive assessments
• Work as part of an interdisciplinary team
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate documentation, including ICD-10 coding
• Communicate and collaborate with the care team
• Facilitate advanced illness and care planning discussions
• Offer counseling and education to patients and caregivers

• NP Certified in one of the following (FNP, ANP, GNP) or Nurse Practitioner or Physician Assistant
• Access to transportation
• 1+ year of experience (LTC, SNF, Geriatric preferred)

• No on-call
• No weekends
• No holidays required
