Role:
Home Health LPN
Responsibilities- Provide direct patient care using a variety of skills and coordinate services.
- Complete documentation according to policies, procedures, and regulations.
- Follow and implement the plan of care assigned by the Registered Nurse.
- Coordinate homecare services with healthcare team members, patients, families, and community agencies.
- Accept and transcribe physician’s orders accurately.
- Forward clinical notes and progress reports to the Registered Nurse.
- Participate in patient care conferences.
- Completion of nursing education from an approved school.
- Current LPN licensure in the state of practice.
- At least one year of clinical nursing experience, preferably in medical/surgical settings.
- Ability to independently organize work and deliver effective home healthcare services.
- Strong communication skills, both oral and written.
- CPR certification.
