The Cumberland County Community Development Department serves as the lead agency for the Continuum of Care (CoC), overseeing funding and programs that prevent homelessness and provide safe, supportive housing. Our team works closely with local service providers, landlords, and community partners to improve housing stability and quality of life for residents in need. The Social Worker II delivers direct case management, comprehensive assessments, and housing placement support to individuals and families referred through the CoC’s Coordinated Entry system.

- Provide case management for clients referred through the CoC Coordinated Entry system, including intake assessments, service planning, referrals, and follow‑up
- Document all client interactions, services, and progress in the Homeless Management Information System (HMIS)
- Assist clients in overcoming barriers to housing stability, including coordinating with landlords and supporting rental applications
- Provide guidance on budgeting, financial literacy, and accessing supportive services such as behavioral health, employment, or transportation
- Monitor client progress toward housing retention and self‑sufficiency goals
- Conduct regular home or shelter visits to assess living conditions, client needs, and compliance with program requirements
- Maintain accurate and timely case notes, reports, and HMIS entries to ensure adherence to HUD regulations, grant requirements, and departmental policies
- Collaborate with service providers, landlords, and community partners to support clients’ successful housing outcomes
Graduation from an accredited four‑year college or university with a degree in social work from an appropriately accredited institution; bachelor’s degree in a human services field from an appropriately accredited institution and one year of directly related experience; or bachelor’s degree from an appropriately accredited institution and two years directly related experience; or an equivalent combination of education and experience.
Management PreferenceMaster’s degree in a human services field such as social work, counseling, psychology, special education, deaf education, rehabilitation counseling or related curriculum and one year of experience in social work or counseling, preferably with the client population.
- Knowledge of case management principles and practices in housing and homeless services
- Knowledge of HUD CoC regulations, fair housing laws, and local homelessness systems
- Skill in conducting needs assessments and developing individualized service plans
- Ability to establish rapport with clients, including those with behavioral health or substance use challenges
- Skill in communication, problem‑solving, organization, and record‑keeping
- Ability to collaborate effectively with multiple agencies while maintaining confidentiality
- Skill in using Microsoft Office Suite and managing databases, with HMIS experience preferred
