Care Manager; RN

About This Role

The Care Manager (RN) develops, assesses, and facilitates complex care management activities for members with physical needs, aiming to provide high-quality, cost-effective healthcare outcomes.

• Pay: $56,200 - $101,000/year
• Location: Remote, WA
• Schedule: Full Time, Day shift

• Evaluate the needs of the member and recommend/facilitate plans for optimal outcomes.
• Develop ongoing care/service plans and collaborate with providers to address unmet needs.
• Identify problems/barriers to care and provide appropriate care management interventions.
• Coordinate between the member, family/caregivers, and care provider team for person-centered care.
• Provide ongoing follow-up and monitoring of member status and progress towards care plan goals.
• Provide resource support for local services based on assessments and plans.
• Facilitate care management and collaborate with providers to ensure timely access to care.
• Perform telephonic, digital, home, and/or other outreach to assess member needs.
• Collect, document, and maintain member information to ensure compliance with regulations.
• Provide education to members and their families on disease processes and healthcare options.

Education:

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing required.

Experience:

• 2 – 4 years of related experience preferred.

Licenses &

Certifications:

• RN
  - Registered Nurse
  - State Licensure and/or Compact State Licensure required.
