Signify Health is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. You will visit members in their homes, review medications and history, perform physical exams, and, when ordered, run diagnostic tests using portable tools and an iPad workflow.
The role is part-time/per diem with an hourly base rate plus incentives, malpractice coverage, and supplies.
