Job Description Summary

Full time opportunity, with one Saturday per month. The Primary Care Prenatal Nurse is a registered nurse who specializes in providing high quality comprehensive care to individuals throughout the lifespan. This includes childbearing individuals, pregnant and postpartum persons, as well as providing support to their families. As part of a clinical team, the nurse works at the top of their licensure to enhance the safety, effectiveness, and efficiency of care through both the performance of professional nursing responsibilities and the delegation of technical and clerical functions as appropriate to medical assistants.

The Prenatal Nurse is key to providing support for our providers, safe care for our patients, and ultimately improving clinical patient outcomes. The Prenatal Nurse primarily assists in the care coordination our perinatal patient's need, while serving as a teacher, advocate, and navigator for these patients within the context of the care team. Assumes responsibility, in collaboration with providers to coordinate care, support transitions between levels of care, and counsel, educate, and support patients in improving health status.

Applies principles of advanced access, planned care and the chronic care model, and rapid cycle performance improvement using the PDSA approach  knowledge, skills, and expertise a good prenatal nurse brings to the job can be invaluable to expectant mothers and their babies.

Role and Responsibilities
  • Support primary care of patients under planned care model, including pre‑visit planning, daily huddles, and execution of nurse‑led and nurse‑assisted visits.
  • Triage patients (both in person and over the phone) based on clear triage protocols.
  • Collaborate with PCP to identify needs for preventive care, evidence‑based chronic disease care, and care coordination.
  • Provide general and specific health care education during patient visits and by telephone.
  • Work with patients to identify and monitor self‑management goals.
  • Collaborate and coordinate care with other community based providers. Including attending and presenting at Continuation of Care Collaboration meetings with the local hospital child birthing center.
  • Follow up on emergency room visits and hospital discharges. Ensuring OB discharge follow up has occurred.
  • Assist providers with exams, treatments, and procedures as needed.
  • Perform placement and monitoring of patients for prenatal NSTs.
  • Ensure safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.
  • Administer medications, including immunizations, at the order of prescribing provider and under standing orders as established by CHCI.
  • Adhere to CHCI protocols regarding documentation of medication, route, dosage, and information provided to patient.
  • Monitor patient response to medication based on specific medication administered; detect, report, and respond to any adverse reaction. Reconcile medications. Provide patient education as appropriate.
  • Perform wide range of nursing activities as needed and after initial prescription by provider such as wound care, HTN follow up, Rhogam administration, gestational diabetes education, etc.
  • Perform a wide range of nursing visits under CHCIs standing orders or delegated visits by the provider. Examples include prenatal intakes, uncomplicated UTIs, titration of long‑acting insulin, treatment of vulvovaginal candidiasis, and pregnancy testing.
  • Accurately document all aspects of patient care in the medical chart on a timely basis.
  • Maintain strict infection control standards in accord with CHCI protocols and universal precautions JC, OSHA and CLIA standards.
  • Act as a resource person for medical assistants regarding patient preparation, needs and procedures needed.
  • Monitor and ensure follow up of all abnormal cancer screening results (PAPs and Mammograms), STIs, and other tests and screens performed at CHCI.
  • Oversee the maintenance of supplies and equipment, ordering supplies as necessary.
  • Maintain compliance with agency mandatory annual training (Patient Safety, Abuse, Age Based Competencies, EOC, Abuse trainings, etc.).
