In-Home Health - Nurse Practitioner or Physician Assistant; Per Diem - Okanagan, WA

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. These visits occur in members’ homes, allowing clinicians to build trusted relationships and deliver comprehensive, personalized health assessments within the comfort of the client’s environment.

• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).

As an in‑home clinician, your primary workplace is the private residence of health‑plan members. Candidates must be able to comfortably and safely perform duties in environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover the Okanagan and surrounding area.

Pay Range: $46.03 – $99.14 per hour. This range represents the base hourly rate for the position; the actual base offer will depend on experience, education, geography, and other factors.

• Hourly base rate plus additional incentives (e.g., per visit completed).
