RN Case Manager III
The RN Case Manager cares for ill, injured, or convalescing clients or persons with disabilities in a hospice care setting. The RN Case Manager assesses client health problems and needs, develops and implements nursing care plans, and advises client on health maintenance and disease prevention, and provides overall case management. RN Case Managers direct and supervise the work of licensed nurses and home health / home care aides.
Must be available to work weekends and after-hours. This role is specific to a scheduling pattern covering 5pm to 8am on weekdays and all hours on weekends. It is a 7-on, 7-off scheduling pattern.
- Consult and coordinate with healthcare team members to assess, plan, implement, modify, and evaluate client care plans.
- Provide basic-to-advanced client care or treatments, such as taking vitals, assessing for and dressing wounds, treating bedsores, giving enemas or douches, immunizations, and performing catheterizations.
- Administer medications and intravenous fluids and monitor client for reactions or side effects.
- Document and record resident observations and health-related information in electronic systems.
- Observe and supervise licensed nurses and nurses' aides or assistants to ensure proper nursing care delivery.
- Coordinates discharge planning and prepares discharge summaries and instructions.
- As needed, collect samples, such as blood, urine, or sputum from patients, and perform routine laboratory tests on samples.
- As needed, measure and record client's vital signs, such as height, weight, temperature, blood pressure, pulse, or respiration.
- As needed, care for client by monitoring health status, feeding, bathing, dressing, grooming, toileting, assisting with mobility, and serving meals and snacks.
- Establish and maintain interpersonal relationships with clients, families, and coworkers.
- Communicate resident observations and health-related information to others member of the care team including coworkers, supervisors, third-party providers, and families.
- Learn and apply Company, Federal, State, and local rules, regulations, and policies.
