The Clinical Coordinator RN will work on-site at DaVita's 6-chair Skilled Nursing Facility dialysis dens. The modality eliminates the need for patient travel to an outpatient dialysis facility. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week, placing a burden on patients and adding avoidable costs to the health care system.

The home site is DaVita SNF Dialysis Fenton 512 Beach St, Fenton, MI 48430.DaVita is seeking a Clinical Coordinator RN to lead dialysis care in a skilled nursing facility. In this role, you’ll oversee patient care delivery, support a clinical team, and ensure treatment meets the highest standards of safety and quality.

* Key Responsibilities:

*** Coordinate care for ESRD patients, including labs, vitals, and outcomes
* Lead and support a team of clinicians and PCTs
* Deliver direct patient care in a skilled nursing facility
* Monitor patient status and adjust care as needed
* Ensure compliance with all clinical and regulatory standards
* Support Administrative Growth
* Communication with our SNF partner
* Coaching teammates on the resolution of issues and escalating to Regional Operations Manager as appropriate.
* Training teammates to include in-services
* Managing scheduling and call-ins
* Staffing the floor ~50% of the time

* Qualifications:

**
* ** RN license in the state of practice**
* ** Minimum of 18 months of RN experience**
* ** Minimum 6 months of dialysis experience
*** CPR certification
* Charge RN readiness approval required
* CNN or CDN certification preferred
* Supervisory experience preferred
* Proficiency in MS Word and Outlook
