The purpose of this position is to support providers and patients to ensure patients are receiving the highest quality of care. Primary job duty is quality reporting, patient outreach, and care team collaboration. This position will work with administration, providers, nursing, and patients, and is highly linked to quality, independent work. The Panel Quality Assistant is an extension of the primary care team and helps identify patients in need of preventive and chronic disease management services.

This role reviews reports, works with the care team to address opportunities, and provides patient outreach. The Panel Quality Assistant has contact with patients, providers, nursing, support staff, and administration. Effective interpersonal and communication skills are essential for this position. The Panel Assistant must be able to follow instructions of licensed personnel and work cooperatively with others, exercise initiative, and judgement.

Responsibilities
  • Recommends, documents, and executes standard protocols for closing gaps in preventive and chronic disease management services on behalf of practices.
  • Accesses practice medical record, health plan and physician organization registries for the sole purpose of identifying patient level gaps in care.
  • Accesses practice EHRs to identify patients that have appointments scheduled, have been or need to be seen for care gap closure.
  • Prepares reports to provide lists of open gaps and patients that are due for services.
  • Communicates with practice office managers and other staff on how to increase quality scores with better documentation and gap closure.
  • Assist IT and health plans with audits by accessing practices EHR’s and/or reaching out to practices for proper documentation.
  • Assist in creating policies, protocols, and workflows to implement quality work into care teams.
  • Provide education on reports and workflows to providers and support staff to implement gap closures within the care teams.
Shifts

80 Hours Biweekly, Day Shifts, Monday to Friday.

Compensation

Hourly wage range: $38.93 - $49.84. Please note that final compensation will be determined based on experience, qualifications and internal equity considerations.

Qualifications

Requirements are representative of minimum levels of knowledge, skills, and/or abilities.

Education / Special Skills
  • Graduate of an accredited school of nursing.
  • Proficient verbal and written communication skills.
  • Demonstrated ability to perform within the registered nurse scope of practice.
  • Current Healthcare Provider-level CPR certification upon hire or must be obtained within six months.
Licensure / Certification
  • Current Minnesota RN Licensure

Successful completion of the pre-employment health screening process required.

Must be able to attend the work site per schedule. Opportunities for some hybrid work.

Proficient and professional communication, both verbal and written required.
