Salary Range: $24.40 – $35.30 per hour
Work Hours: Rotating shift; evenings, weekends, and holidays as needed
Travel: Yes – transportation and accompaniment to medical appointments, community outings, recreational activities, and spiritual events
Primary Responsibilities- Participate in the development, refinement, and implementation of individualized program plans that help individuals safely achieve their goals.
- Provide work direction, guidance, training, and feedback to staff to ensure the highest standards of service delivery.
- Deliver direct care to individuals, including bathing, toileting, hygiene, medication and treatment administration, escorts, mobile supports, and behavior interventions.
- Contribute to the safety and security of the site, the individuals served, and co‑workers.
- 4 years of full‑time direct‑care work experience with persons with disabilities or persons 65 years and older.
- Or a minimum of 50 hours of education and training related to human services and disabilities.
- Or a diploma in a human‑services field (including but not limited to Family Social Sciences, Health Care Management, Special Education, Public Health, Recreational Therapy, Social Work, Alcohol and Drug Counseling, Social Science, Health Science, Psychology) with 3 years of full‑time direct‑care experience.
- Or an associate degree in a human‑services field with 2 years of full‑time direct‑care experience.
- Or a bachelor’s degree in a human‑services field with 1 year of full‑time direct‑care experience.
- Valid driver’s license.
- At least 18 years of age.
- Experience working effectively with individuals from diverse cultural backgrounds.
- Lead or supervisory experience in a direct‑care setting.
- Experience writing, modifying, or evaluating program plans.
- Experience managing individual or home accounts.
- Resume must clearly describe experience, including start and end month and year for each position.
- Reference and background checks will be conducted prior to appointment.
