RN Case Manager
The RN Case Manager is responsible for planning and managing the nursing care of an assigned caseload of patients in home or long-term care settings. In collaboration with the interdisciplinary team, the RN Case Manager ensures that patient- and family-directed goals are achieved using a holistic, patient-centered approach. The role includes providing education, facilitating communication, and coordinating care across various sites.
Flexibility in work location across care settings is required.
Essential Functions
- Assess and identify patient and family goals and needs.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Participate in team meetings and ethical discussions.
- Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
- Coordinate timely and effective responses to changing or urgent patient needs.
- Maintain accurate, timely, and compliant documentation.
- Support and educate team members, patients, families, and the broader community.
- Participate in internal quality improvement initiatives.
- Facilitate interdisciplinary group (IDG) meetings in the absence of the manager.
- Perform other duties as assigned.
Qualifications
Knowledge and Critical Skills
- Effective verbal, written, and non-verbal communication skills.
- Understanding of hospice and palliative nursing principles.
- Cultural competence and ethical decision-making in care delivery.
- Ability to set professional boundaries and resolve team conflicts.
- Strong critical thinking and clinical judgment abilities.
- Proficient in time management, organization, and accountability.
- Knowledge of infection control and standard precautions.
Experience
- Minimum of one year of Registered Nurse experience.
- Preferred:
One year of experience in a hospital medical-surgical or oncology setting. - Acceptable alternative:
Five years of Licensed Practical Nurse experience within the past five years, including one year of hospital medical-surgical or oncology experience
License / Certification
- Current Registered Nurse license in the State of Florida.
- Valid CPR certification.
- Current driver's license with an acceptable driving record.
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
Other Requirements
- Medical clearance for communicable diseases.
- Up-to-date immunizations (e.g., TB, Hepatitis, Flu).
- Ability to work extended and irregular hours as needed.
- Ongoing participation in professional development and continuing education.
