Position: Patient Care Unit Coordinator - Pediatrics (Hospice)

Description

Job Summary:

The Patient Care Unit Coordinator assists the RN Clinical Coordinator(s) in providing direction and clinical support for a specified unit and assigned staff. This position performs administrative support activities required for proper transcription of medication, treatment orders, documenting and coordinating patient care. The Patient Care Unit Coordinator will maintain clinical records of the hospice patients and will function as a liaison between other team members assisting with day-to-day management duties.

Director of Pediatrics & Specialized Services and the RN Clinical Coordinator(s)

FLSA Status: Non-Exempt/Hourly

Requirements

Job Duties and Responsibilities:

Essential Job Functions:
  • Fosters and maintains an effective working relationship between co-workers, patients, and other professional members of the community
  • Assures patient records are complete and all required documentation is filed in a timely manner
  • Enters patient data and visits into the EMR system
  • May order and monitor supplies and medical durable equipment
  • Responsible for faxing paperwork required by facilities and PDN Companies
  • Closes open visits in the EMR for billing
  • Assists in submission of claims for auths
  • Checks Medicaid monthly
  • Assists with Medicaid authorizations
  • Pushes orders, plan of care summaries and CTIs in EMR
  • Sends orders/referrals to providers for signature
  • Submits and processes pediatric referrals
  • Prepares documentation for interdisciplinary team meetings as directed
  • Manages and processes referrals as needed, including obtaining provider signatures
  • Obtains and responds to requests for records
  • May be responsible for running pharmacy reports for patient recertification
  • Fills in on the switchboard for lunches, absences and time off as requested
  • Other duties as assigned

  • Adheres to patient rights, abuse reporting, and confidentiality policies
  • Attends scheduled staff meetings and in-service training
  • Adheres to departmental dress code requirements related to uniform, personal grooming, and other attire
  • Participates in Performance Improvement activities as requested
  • Demonstrates reliability by making every attempt to report to work on time, providing proper notification for absence or tardiness and follows company policies related to time off requests
  • Demonstrates effective time management skills by coordinating activities to achieve maximum
  • productivity and efficiency, willingness to adjust or revise work schedules as requested
  • Assumes responsibility for on-going personal development and continuing education
  • Seeks guidance from a higher authority as needed
  • Physical/Safety Requirements:
  • Must have the strength and endurance required for standing, walking, lifting, bending, and pushing on a consistent basis up to 25 lbs.
  • Utilizes appropriate durable medical equipment as needed in a safe manner
  • Uses appropriate body mechanics as needed for safe patient care
  • People in this position may be subject to potential exposure to infectious waste, disease, and other conditions, including TB, HIV and Hepatitis B
  • Utilizes appropriate PPE as needed
  • Follows the designated plan of action in the event of a fire or other emergency/disaster
  • While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work-related stress, and driving hazards
  • An individual in this position may be required to drive to meetings with clients or health care professionals, as well as patients’ homes.
  • The noise level in the work environment is usually minimal

    Qualifications and

    Experience:
  • Two years’ experience as a Licensed Practical Nurse, Medical Assistant or Certified Nursing Assistant in hospice, home health or geriatric care preferred
  • One year of experience in an office setting, medical office experience required
  • Demonstrated expertise with Microsoft Office
  • Excellent interpersonal and communication skills
  • Must be able to work with a variety of personalities
  • Successful completion of a background check pursuant to NRS 449.188 and NRS 179A.100 along with a pre-employment drug screen and physical, TB testing, previous employment and reference checking, and an MVR (may include verification of education)
  • CPR/First Aid certified
  • Must be able to work as a team
  • Ability to function with limited supervision and accountability to meet assigned projects and timelines
  • Must be able to handle frequent interruptions
  • Must be able to cope with emotionally charged situations encompassing, patients, families, and other caregivers
  • Must have reliable transportation
