Center Well Home Health is seeking a Medical Social Worker to join our caring team. Reporting to the Clinical Manager, you will evaluate and develop a personalized plan of care addressing emotional and social needs of home health patients and supervise the Social Worker Assistant when involved in the patient’s plan of care.
You will collaborate with the interdisciplinary team to coordinate care, support discharge planning, and ensure compliance with company policies and regulations.
